Error Loading Survey: You have already completed this survey

If users are having problems accessing surveys attached to DocRead tasks and receive the following error:

Please check that the survey has been created using the option “allow multiple responses” has been used as shown below:

If a user has already completed the survey or quiz for whatever reason (it may be attached to other tasks they have been asked to complete or they attempted the survey but failed to achieve the minimum pass rate and so could not complete the DocRead task) they will not be allowed to retake the survey unless the multiple response box is checked.
